Set the Instant Pot to the sauté setting. Add the ground beef, onion, garlic, Italian seasoning, salt, pepper, and red pepper flakes (if using). Cook until the beef is browned, stirring occasionally.
Turn off the Instant Pot and add the macaroni, beef broth, and diced tomatoes. Stir to combine.
Place the lid on the Instant Pot and set the valve to sealing. Select the Manual or Pressure Cook setting and set the timer to 4 minutes.
Once the timer is done, quick release the pressure and open the lid. Stir in the cheddar cheese until melted.
